Basic Legal Requirements
- Must be a resident of state of Alaska
- Must be 18 years of age, unless written consent from a parent or legal guardian is given
- Must have an Alaska government-issued photo identification card
Proof of Qualifying Conditions
- Have any of the qualifying conditions
- Have a signed Department-approved physician statement form stating the qualifying medical condition
- Have medical records proving diagnosis and treatment of the condition
Complete the Paperwork
- Complete the application form and gather the other required documents, together with a check or money order for the required fee, and mail to:
